[Archivesspace_Users_Group] Migration from 1.2 to 1.4

Hi Chris, 
As a workaround, I simply removed those three tables, ran the script, and the three tables were created. Is this a known issue with the upgrade process?

After you ran the setup-database script, did the telephone, rde_template, and default_value tables get created?     

If so, you should be good if the schema_info table has a value 59.

                                 telephone                                rde_template                                default_values                                
                                 I temporarily removed these tables from the database and was able to run the upgrade process successfully.
